Người mắc bệnh đái tháo đường (ĐTĐ) týp 2 thường hay bị cao huyết áp (với tỷ lệ 60-80%). Cao huyết áp kéo dài sẽ làm tổn thương các mạch máu nhỏ và các mạch máu lớn, nếu không được điều trị sẽ gây ra các bệnh lý ở mắt, thận, tim mạch và có thể dẫn đến đột quỵ.

Kiểm soát huyết áp cũng như kiểm soát đường huyết và lipid là những vấn đề rất quan trọng đối với người mắc bệnh tiểu đường týp 2, giúp làm chậm sự tiến triển của bệnh ĐTĐ và ngăn ngừa những biến chứng bệnh lý có thể xảy ra.

Huyết áp là áp lực của máu lên các thành mạch. Một người được cho là cao huyết áp khi HA tâm thu >140mg (HA tâm thu là huyết áp đo được khi tim co bóp máu đi) và HA tâm trương >90mg (HA tâm trương là huyết áp đo được khi máu trở về tim : >140/90mg). Huyết áp của người khỏe mạnh bình thường là 120/80mg. Đối với người mắc bệnh ĐTĐ týp 2 mục tiêu điều trị là đưa huyết áp ở mức <130/85mg. Để điều trị cao huyết áp cho người ĐTĐ týp 2 có hai phương pháp là dùng thuốc và không dùng thuốc.

Phương pháp không dùng thuốc:

– Kiểm soát cân nặng, tránh béo phì.

– Tập thể dục điều độ với cường độ trung bình như đi bộ, bơi lội…

– Thư giãn đầu óc, tránh căng thẳng stress.

– Hạn chế uống rượu.

– Tốt nhất nên bỏ hút thuốc.

– Chế độ ăn kiêng mặn và chất béo.

Phương pháp dùng thuốc:

Có rất nhiều nhóm thuốc dùng để điều trị cao huyết áp như: thuốc lợi tiểu, thuốc chẹn thụ thể, thuốc đối kháng canxi, thuốc giãn mạch, thuốc tác động lên hệ thần kinh trung ương, thuốc ức chế men chuyển… Hầu hết các nhóm thuốc cao huyết áp đều có những tác dụng phụ. Người thầy thuốc phải lựa chọn nhóm thuốc nào có tác dụng tối ưu và hạn chế tác dụng phụ đặc biệt là những tác dụng phụ trên thận để điều trị cao huyết áp cho người bệnh ĐTĐ týp 2. Nhóm thuốc ức chế men chuyển (ACE) như thuốc captopril, enalapril… là lựa chọn hàng đầu của các thầy thuốc do vừa có tác dụng hạ huyết áp vừa có tác dụng bảo vệ thận.

Thuốc ức chế men chuyển – (ACE):

ACE (Angiotensin converting enzyme) là một peptidyl dipeptidase đóng vai trò xúc tác chuyển đổi ngiotensin I thành angiotensin II. Angiotensin II là chất co mạch nội sinh và có vai trò kích thích vỏ thượng thận tiết ra aldosteron gây giữ muối và nước. khi vào cơ thể, thuốc sẽ ức chế ACE ngăn chặn sự hình thành angiotensin II nên gây giãn mạch hạ huyết áp, đồng thời cũng ngăn chặn sự kích thích tiết ra aldosteron ở vỏ thượng thận nên có tác dụng bảo vệ thận.

Vì vậy, trong điều trị cao huyết áp ở người bệnh ĐTĐ týp 2, các thầy thuốc sẽ ưu tiên lựa chọn nhóm thuốc ức chế men chuyển vì vừa đạt hiệu quả vừa an toàn cho bệnh nhân. Ngoài ra trong trường hợp cần thiết, các thầy thuốc có thể phối hợp nhóm thuốc này với các nhóm thuốc hạ huyết áp khác để đạt hiệu quả điều trị như mong muốn.
